

Our Maternal Health Equity Model
The Foxx Foundation implements a community-based maternal health equity model designed to improve outcomes, reduce disparities, and expand access to culturally-responsive perinatal care for underserved families.
In partnership with Mind Body Birth Wellness and Birthing Center, our work centers relationship-based, continuity-led care across the reproductive, pregnancy, birth, postpartum, and newborn continuum.
We believe every family deserves access to respectful, individualized, and supportive care regardless of income, identity, or background.

Why Community-Based Midwifery Care Matters
Black, Indigenous, Queer, and low-income families continue to experience disproportionately poor maternal and infant health outcomes due to systemic inequities, fragmented healthcare systems, financial barriers, and lack of access to culturally-responsive care.
Research consistently demonstrates that midwifery-led, continuity-based care models improve maternal and infant outcomes, reduce unnecessary interventions, and strengthen client and family wellbeing — particularly for historically underserved communities.
Despite this evidence, access to community-based midwifery care remains limited for many families due to structural and reimbursement barriers.
The Foxx Foundation works to reduce these barriers and expand access to relationship-centered care models rooted in dignity, trust, informed decision-making, and continuity of support.
Core Components of Our Model
Our maternal health equity model is rooted in the following principles:
Relationship-Centered Care
We prioritize trust, continuity, informed decision-making, and individualized support throughout the reproductive and perinatal journey.
Community-Based Access
We work to reduce financial and systemic barriers that prevent underserved families from accessing culturally-responsive perinatal care.
Continuity of Care
Families receive continuous support across prenatal, birth, postpartum, newborn, and reproductive health services, helping improve communication, trust, and outcomes.
Holistic + Integrated Support
Our model recognizes that maternal and infant wellbeing are influenced by physical, emotional, mental, social, and environmental factors.
Services include:
Prenatal and postpartum support
Lactation support
Reproductive health education
Care coordination and referrals
Community resources and supportive services
Collaboration + Appropriate Medical Integration
Our model supports appropriate consultation, collaboration, and transfer when higher levels of medical care are needed.
